Step 2: Submit Application Materials

  1. Complete the Application Packet

    • The following documentation will also be needed

      • Child's original birth certificate

      • Copy of baptismal certificate (if applicable)

      • Your child's most recent report cards and standardized test scores

      • 504/IEP (if applicable)

  2. Applications available in our main office and here:

  3. Application Fee:

    • $45/per child until February 10th, 2024

    • $60/per child from February 11th - March 7th, 2024

    • $150/per child after March 7th, 2024
